What should I do if I receive a damaged ticket from a Hoosier Lottery vending machine?
If your ticket is damaged, first ask the retailer for reimbursement. If they can’t help, submit the ticket with a completed
Winner Claim Form to the Hoosier Lottery, even if you think it’s not a winning ticket. You can mail the form and ticket or bring it to one of our
Prize Payment Claim Centers in person. Please note that you will not receive same-day payment for damaged tickets; we will contact you with a resolution.
What if I have a winning ticket, but it is showing at retail as already redeemed?
To resolve this, follow these steps:
- Sign the back of your winning ticket.
- Complete the Winner Claim Form.
If mailing the form, include a brief explanation of the issue (e.g., "I attempted to redeem this ticket, but it says it was previously claimed").
