Daily 3 and Daily 4 with SUPERBALL Evening Drawing for 3/19/2022

The Hoosier Lottery inadvertently displayed an incorrect SUPERBALL winning number for the Saturday, March 19, 2022, Daily 3 and Daily 4 evening drawings. The correct winning numbers for the March 19 Daily 3 evening drawing are 4-1-0 and the SUPERBALL number is 8. The correct winning numbers for March 19 Daily 4 evening drawing are 3-2-9-4 and the SUPERBALL number is 8. Following the drawing, the incorrect SUPERBALL number posted by the Hoosier Lottery was 3. Due to the error, both sets of SUPERBALL numbers, 8 and 3, will be honored for prize payment.

Winning tickets with Superball number 3 may be redeemed at a retail location.

For winning tickets with Superball number 8, claims can be made via mail or in-person at any of our three prize payment locations.

To claim a Daily 3 and Daily 4 with Superball ticket from 3/19/22 by mail

  1. Sign and complete the information on the back of your winning ticket 
  2. Print and fill out the winner claim form
    • Leave lines 1-6 blank
    • Complete lines 7-19 
    • Sign and date the form on the lines indicated
    • Please note: the claim form must be completed as directed or your claim form may be returned to you. 
  3. Fill out IRS FORM 5754 (if more than one person is claiming the prize) all parties must submit a copy of their ID.
  4. Make a copy of the front and back of your ticket(s) for your records.
  5. Staple the original D3 or D4 with Superball ticket and copy of your valid Government Issued Photo ID to the original (signed) claim form and mail to the address listed below. 
  6. Claims will be processed in the order received and a check mailed to the address listed on the claim form.
  7. Some tickets may be subject to additional questions as part of the normal claims process.
  8. We suggest mailing items via UPS, FedEx or USPS Certified. The Hoosier Lottery is not responsible for lost or stolen tickets, and we do not reimburse postage charges associated with any of these services. To claim a Daily 3 and Daily 4 with Superball ticket from 3/19/22 in-person

To submit a claim in person you must have the actual D3 or D4 with Superball ticket. Please note that claims will not be paid the same day.

  1. You must sign and complete the information on the back of your winning ticket
  2. Complete a winner claim form  
  3. Submit a copy of your government issue photo ID or the original and a copy will be made for you.
  4. A copy of your ticket will be made and provided to you. Claims will be processed in the order received and a check will be mailed to you.
  5. Some tickets may be subject to additional questions as part of the normal claims process

Lost Ticket Instructions

  1. If you no longer have the ticket you purchased, please follow the steps listed in the DAILY 3 AND DAILY 4 WITH SUPERBALL Affidavit link shown below.    
  2. A separate affidavit must be submitted for each missing ticket. Affidavit will only be accepted at the address listed on the bottom of the form.
